摘要： Objective: This meta-analysis aimed to quantify sensitization rates following allograft usage and determine whether allografts have an increased risk of long-term sensitization compared to alternative therapies in burn patients. Methods: Systematic review, meta-analysis and meta-regression of post-operative sensitization in burn patients were performed following Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidance (Prospero registration: CRD42024497137). Database searches were conducted on MEDLINE, Embase, CENTRAL using ProQuest Dialog. We included all studies reporting post-operative sensitization in burn patients that were not case series or singular case reports. Meta-analysis was used to compare the risk of sensitization for allograft versus alternative therapies. Results: Six studies (Allograft Patients: 71;Non-Allograft Patients: 95) were included (n =166). Overall, 73 % (CI: 39 %-92 %) of burn patients were sensitized following allograft. Patients receiving allograft were at a significantly increased risk of sensitization (OR 13.14 [95 % CI: 1.32-131.00]). However, on sub-group analysis, when directly comparing patients who received allograft and transfusion versus those who received transfusion alone, there was not a significantly increased risk of sensitization (OR 7.68 [95 % CI:.61-95.86]). Conclusion: Allograft is associated with a significantly increased risk of sensitization in burn patients. However, it is not clear whether burn patients already receiving transfusion will have an increased risk of sensitization from adding allograft. Further studies are required to demonstrate the multifactorial causes of sensitization within the burned population to explain the significance of the contribution from allograft and/or transfusion reported within the literature. 摘要： Objectives: This study aimed to determine the diverse inborn and adventitious variables that contribute to the rise of Ankylosing Spondylitis (AS) and to elucidate the genotypic study of HLA-B*27 alleles along with suballeles and in-silico inhibition of their respective abnormal receptor proteins by natural compounds. Methods: Case-control study was piloted. Allele-specific DNA-based HLA typing was performed after DNA extraction. Patient questionnaires and Molecular docking was applied to identify AS prognosis and potential HLA-B*27 inhibitors respectively. Results: Results revealed a 72.72% prevalence of HLA-B*27 alleles in patients versus 9.09% in controls. Suballeles HLA-B*27:02, 04, and 05 were identified in 87.5% of patients but were absent in controls. The chisquare (chi 2) values for HLA-B*27 alleles and sub-alleles were significant, with p-values of 0.0024 and 0.0220, respectively. The study found no significant association of AS with gender, age, marital status, or environmental factors, but a strong association with family history of back pain, elevated CRP, ESR, body inflammation, and uveitis. In silico analysis identified Rutin, curcumin, and coumaroylquinic acid as natural compounds with the highest binding affinity to HLA-B27 chains A and F, suggesting their probability to modulate the structure and function of HLA-B27 proteins. Conclusions: AS is more prevalent in individuals with family history of backache, uveitis and elevated inflammatory markers, Sub-alleles of HLA-B*27 should be used as diagnostic tools alongside alleles, as they were found only in patients, not in healthy individuals. Furthermore, Rutin, curcumin, and coumaroylquinic acid may temper the function of HLA associated with AS.

摘要： This study investigates potential epitopes in the glycoprotein and RNA of Chandipura vesiculovirus (CHPV) using MHC Class I (HLA-A0201) and MHC Class II (DRB1_0101) molecules with 3D structures derived from x-ray crystallography. Computationally derived peptides were mapped and subjected to in silico docking, revealing promising targets for CD8+ cytotoxic and CD4+ helper T cells. Key factors analysed include solvent accessible surface area (SASA), Debye-Waller factor (B-factor), and polar bond interactions. Post-docking, removal of N-acetylglucosamine (NAG) increased peptide stability and reduced B-factors, while SO4 presence had minimal impact. SASA values increased by up to 237.5% with MHC Class I, and RNA docking with MHC Class II displayed mixed SASA changes. Polar bond interactions also increased post-docking, indicating the strong potential of identified CHPV epitopes.

摘要： Background: Hyperuricemia (HUA) is a metabolic disease disturbing human health caused by the overproduction or underexcretion of uric acid (UA). Astragalus is the root of Astragalus membranaceus (Fisch.) Bunge, has notable regulatory effect on chronic nephritis, proteinuria and spontaneous sweating, suggesting it could be a potential anti-HUA agent. However, limited research has been conducted on its anti-HUA effect and mechanism. Methods: The present study performed untargeted and plasma metabolomics of Astragalus extract to identify the main constituents that can be absorbed and exert effect in mice, and further investigated the underlying mechanism by enzyme activity assay, Western Blotting and molecular docking. Results: The results showed that Astragalus flavone extract inhibited UA synthesis by binding to XOD to hinder substrate binding and inhibiting xanthine oxidase (XOD) protein expression, inhibited JNK/AP-1/NLRP3/IL-1 beta signaling pathway to alleviate prolonged HUA-induced inflammation and abnormal UA metabolism, and protected the kidney by reducing serum renal function index and improving renal tissue atrophy, fibrosis and tubular dilatation both in vitro and in vivo. Besides, glycitein and isoformononet were identified as the main flavones in Astragalus extract absorbed into the bloodstream of mice, isoformononetin was found to inhibit UA synthesis by direct binding to XOD, and glycitein was found to interact with c-Jun to facilitate UA excretion and inhibit inflammation. Conclusion: This paper represents the pioneering investigation that firstly identifying two flavonoids of Astragalus extract that can be absorbed to fight against HUA, and elucidating their diverse molecular mechanism by targeting JNK/AP-1/NLRP3/IL-1 beta signaling pathway, UA metabolism and kidney protection.

摘要： HLA (human leukocyte antigen) typing is performed on both organ donors and recipients to ensure compatibility and thus maximize graft and patient survival. In the US, the final assigned HLA typing is then entered, often manually, into the DonorNet system managed by United Network for Organ Sharing (UNOS), where it is used to generate a list of potential recipients. For deceased donor organs, HLA typing is most often performed via real-time PCR (rtPCR), which is rapid and robust but requires human interpretation. Human error in that interpretation or in the data entry process (regardless of whether the typing is generated by rtPCR or sequencing) can have severe consequences including organ wastage due to inappropriate matching and graft rejection. UNOS requires laboratories to validate the accuracy of their DonorNet submission. To address this need, we have developed DonorCheck (https://***/PankratzLab/DonorCheck/), an open-source and userfriendly software application written in Java that validates DonorNet field entries against the original donor HLA typing source report. DonorCheck helps HLA laboratories independently validate their interpretation of rtPCR data (including accounting for allele, P-group, and serotype lookup) and the HLA data entry into DonorNet, reduce data entry errors, and enhance patient safety.
